The old thread hit 11 pages, kind of unwieldy, so I am starting a new one.

"The patient became very lethargic and sleeping excessively more than usual."

zzzz

This doc also says "peewid" after evwry sentence.

Sounds a lot like one I have who likes to say, "The patient is feeling fatigued, very tired, low energy." Sheesh, we get it already!!!

"The plan was discussed with the patient. He was agreeable and all answers were questioned."

Sounds like my kind of doc. \:\)

This one made me giggle:
"The patient is married but denies taking any illegal drugs."

"The patient denies any loss of conscience." (I'm certainly glad to hear THAT, lol)

I have a nurse practitioner who keeps saying "underlining" instead of "underlying." Drives me absolutely crazy. She also says "thyromeglia" and "organomeglia."

I had a cardiologist today who tried three times to pronounce "ophthalmopathy" and he finally gave up.

One doc keeps saying his patients are "invertebrate" smokers instead of "inveterate." That paints a pretty picture, huh?

And then there's the rheumatologist who says "tinazidine" instead of "tizanidine."

I don't mind that they can't pronounce words common to other disciplines, but when they can't pronounce their own terms, it does make me wonder.

I can see why it's a common practice to take a time out before surgery to make sure they have the correct patient and are operating on the correct thing. But it does seem that it would be slightly more effective if they did this before the patient was put under anesthesia and could tell them who/what.

"the patient denies nausea, vomiting, or diuresis."

talking about a cardiac patient, he meant diaphoresis. However, the picture of a patient trying to take care of all 3 things at once is a litle scary.

Verapamil was discontinued and he was started on metoprolol succinate, which he seems to be tolerating, though he does not have any breath sounds and he doesn’t have any air moving.

\:o What??? WOW!

This podiatrist quotes the patient directly:

My nails hurt and this thing on the bottom is really barking.

woof, woof!

Pain management consultation copied to an insurance company. Respectfully yours, blah, blah... The doc then said, "Actually, take that out. I don't really respect them."

I have a podiatry resident who says "whiff" instead of with - drives me nuts. Just once I would love to type it as he says it but I suppose that would be mean.

A veteran gastroenterologist says "in lieu of" when he means "in view of" constantly, which we all correct, of course.

An OB/GYN from another country says "comma" after every three or four words. Not quite sure why - and who taught him to do that???

We are in the process of going to voice recognition, so I can't wait to see how some of these reports are going to look... YIKES!
