Sounds like this PT uses some kind of form or text boxes.
The PT I used to do was charged by the line, but it was mostly in letter format with no charts. I did do some functional assessment forms for her, and those were charged by the line also. I tended to charge more when I had to do jumping around/tabbing/et cetera.
