Just curious, is there is a preferred way to transcribe ICD codes?
This is a physical, occupational, and speech therapy account I work on and they have recently began dictating the ICD-9 codes after the medical diagnosis. All of the magazines / reference books I read all show the diagnosis with the code in parentheses after it. Do I need to type out ICD-9 if it is dictated?

Do I need to type out ICD-9 if it is dictated? Usually, if a doc dictates it, they're trying to prevent problems they've had in the past with incorrect coding and insurance co. denials. I especially encounter this when I get stuff from the wound care and hyperbarics depts. Unfortunately, MTs aren't exactly trained to catch coding errors (you know how well they spell for us). Still--of course you would transcribe it. Why would you not?

I *think* she meant did she need to put the label "ICD-9" on the numbers they dictate.
i.e. DIAGNOSIS: Cerebrovascular accident (xxx.x) versus Cerebrovascular accident (ICD-9 xxx.x)
I don't think the label is necessary, especially if not dictated. I do like the parentheses. If no account specific ruling, you are really free to make your own format, though.

That is what I meant tropsicle. Thanks for both replies though. I will transcribe as cerebrovascular accident (xxx.x). This is how I have been doing it, however, just wanted another opinion and wasn't sure if the BOS had a rule about it and I could not find it.

There are also ICD-10 codes, so if ICD-9 is dictated, I would definitely type it, but if not, then I definitely would not type it. I don't know what the differences are in the codes, but that's not an MT's concern anyway.

Just out of curiosity, does a doctor only use one set of codes, either just ICD-9 or ICD-10?

You probably should ask down the page in the coders' forum, but I believe the US uses ICD-9 predominantly, and is supposed to transition to ICD-10 to catch up with the rest of the world--hopefully before ICD-11 comes in 2010-2015.
I don't believe you would encounter a dictator using both, and they're probably limited by what their facility uses. I'm with Deb--safest to transcribe as dictated because they might know what they mean here and now, but when someone's looking through old charts down the line, they shouldn't have to do mental gymnastics trying to figure it out.

Adding the ICD-9 would be a style issue you would need to check with your account specs. All coding and billing numbers are easily figured out by the digits.

I am so sorry, I guess I didn't make myself clear. Adding the word "ICD-9", would be a style issue. Meaning what the boss wants, the boss gets.
